EZ TelePsych Policy: Patient–Provider Agreement for the Use of Controlled and Non-Controlled Medications
This Agreement outlines the responsibilities and expectations between the patient and the prescribing provider regarding the safe use, management, and monitoring of prescription medications, including controlled substances (Schedules II–V) and other regularly prescribed medications.
All policies described below are established to ensure patient safety, clinical compliance, and quality of care, in full alignment with applicable state and federal regulations.

1. Appointment and Medication Management
To maintain safe and effective treatment:
-
The patient must schedule and attend regular medication management appointments for evaluation, follow-up, and refills.
-
Medication changes, dosage adjustments, or refills will only be addressed during scheduled appointments with the prescribing provider.
-
Missed or delayed appointments may result in an interruption or discontinuation of medication therapy until a follow-up visit is completed.
2. Prescriptions Outside of Office Hours
EZ TelePsych does not issue or refill prescriptions outside of regular business hours, on weekends, or during holidays.
Patients are responsible for monitoring their medication supply and requesting appointments well in advance of running out.
Medication refills should not be requested from emergency rooms or urgent care settings unless there is a verified medical emergency.
3. Lost, Stolen, or Misused Medication
In the interest of patient safety and responsible prescribing:
-
Medications or prescriptions that are lost, stolen, destroyed, expired, or otherwise misplaced or misused will not be replaced or reissued.
-
A one-time replacement may be considered only if the prescribing provider determines it is clinically necessary and in the best interest of the patient.
-
Any such exception will be documented in the patient’s medical record.
-
Requests for early refills or replacement prescriptions that raise concern for potential misuse or noncompliance will be denied.
4. Refill and Follow-Up Policy
To ensure appropriate monitoring and continuity of care:
-
Controlled Medications: Patients must be seen for a follow-up visit at least every 30 days before any further refills are issued.
-
Non-Controlled Medications: Patients who are stable may receive a 60- or 90-day prescription, as determined by the prescribing provider.
-
Providers may adjust refill intervals or require additional visits based on clinical judgment or safety considerations.
5. Diagnostic and Laboratory Monitoring
Providers may require diagnostic testing or laboratory work—such as urine drug screening, metabolic labs, or other assessments—to ensure safe medication use and to monitor patient health.
Failure to complete requested testing may result in delay or discontinuation of prescriptions until the required evaluations are completed.
6. Medication Safety and Compliance
Patients are expected to:
-
Take medications only as prescribed.
-
Store all medications securely to prevent loss, theft, or misuse.
-
Avoid sharing or selling any prescribed medication.
-
Communicate openly about side effects, changes in symptoms, or other concerns related to their treatment.
7. Provider Discretion
The prescribing provider reserves the right to:
-
Modify, deny, or discontinue any medication if there is evidence or reasonable concern of misuse, abuse, diversion, or noncompliance.
-
Require more frequent visits, additional testing, or treatment plan changes based on patient response and safety.
-
Refer the patient to specialized care or discontinue controlled-substance prescribing if deemed clinically necessary.
Continued treatment is contingent upon adherence to these policies and the provider’s professional judgment regarding the patient’s safety and clinical progress.
